How to Enroll with Us

We’ll collaborate with you to develop an Individualized Service Plan (ISP) that outlines your goals and the services needed to support independence, skill-building, and community integration.
Step 3: Begin Services
Once your plan is in place, services will commence, focusing on personalized support, life skills development, and social engagement.
Funding & Eligibility
Medicaid Waiver (DD Waiver)
Our services are available to individuals receiving support through Medicaid’s Developmental Disability (DD) Waiver program.
Private Pay Services
For individuals not eligible for waiver services or those seeking additional support, we offer flexible private pay options.

✔ What We Offer
Our non-center-based day support services are designed to promote independence, socialization, and life skills through:

✅ Life Skills Development – Budgeting, personal care, meal planning
✅ Health & Wellness Support – Fitness, nutrition, self-care
✅ Vocational Exploration – Job site visits, skill-building workshops
✅ Person-Centered Planning – Support tailored to individual goals
✅ Safe & Compassionate Supervision
✔ Rates & Payment Options
Private pay rates range from $25 to $50 per hour, depending on the level of support needed. This is for individuals who do not have a medicaid waiver.
